A Kokomo woman died following a single-vehicle crash on the east side of Burlington Thursday afternoon.
Dispatchers at the Carroll County 911 Center received a report of a crash on Ind. 22 one mile east of the town at 12:52 p.m.
When first responders arrived at the scene, they found a handicap-equipped 2002 Dodge Entervan had left the south side of 22 and went down an embankment, striking a culvert. That caused the van to roll before going airborne. When it landed, it struck the far side of a small stream and came to rest upside down in the water.
Emergency personnel from Burlington Fire Department and Carroll County EMS had to cut the seatbelt of the driver, Deborah Shay, 56, Kokomo, before they could remove her.
She was pronounced dead at the scene by Carroll County Coroner Jay D. Cree.
The investigation by Carroll County Sheriff Department Sgt. Dennis J. Randle and Deputy Jason Dunning and Chief Paul Redmon of Flora Police is continuing. An autopsy has been scheduled to determine the cause of death.
