LMH Foundation scholarships open

---- — Logansport Memorial Hospital Foundation scholarship applications are now available in the guidance office of any Cass County high school.

Completed applications must be received by LMHF no later than April 1. The LMHF office is in the lower level of Medical Office Building West at Logansport Memorial Hospital. Applications can also be mailed to Logansport Memorial Hospital Foundation, 1101 Michigan Avenue, Logansport IN, 46947.

Only graduating Cass County seniors pursuing careers in healthcare are eligible to apply. Applications must be filled out completely by the candidate. The signature of a guidance counselor is required. Each completed application must include a recent high school transcript and a copy of the applicant’s letter of acceptance to the university or college of his or her choice. Applications received without a transcript or letter of acceptance will automatically be disqualified.

Candidates are also required to submit three letters of reference from individuals, other than parents or relatives, who have known the student for at least four years. At least one reference letter must be from a source outside of school or school-related activities. Incomplete applications will be disqualified.
