Free marketplace enrollment offered at Kokomo hospital

---- — KOKOMO — Community Howard Regional Health, 3500 South Lafountain St., Kokomo, in partnership with Anthem Blue Cross and Blue Shield, will provide free assistance with enrollment in the Indiana Health Insurance Marketplace from 4 to 7 p.m. Dec. 18 in the hospital’s east lobby. Community Health Network will serve as the primary provider network for central Indiana residents who purchase an Anthem health benefits plan on the Indiana Health Insurance Marketplace, or “exchange.”

Those interested in enrolling by phone or in setting up an appointment for Dec. 18 may call 855-365-9300. Representatives are available 8 a.m. to 5 p.m. Monday through Friday. It is recommended that people call to schedule an appointment to reduce wait time.

People attending the enrollment event should bring:

• Social Security numbers (or document numbers for legal immigrants).

• Employer and income information for every member of the household who needs coverage (for example, pay stubs or W-2 forms — Wage and Tax Statements).

• Policy numbers for any current health insurance plans covering members of the household.

• A completed Employer Coverage Tool for every job-based plan for which someone in the household is eligible. This form is available at www.healthcare.gov.
